The Care Advocate works directly with patients (or with their legal representatives) to ensure they have a voice in their care and information to promote informed decision making.
The Care Advocate serves as an educator to internal and external key stakeholders.
In this role you will be responsible for:
Receiving, interpreting and managing patient needs with the intent of assessing for appropriate services Providing consultation on appropriate care services and/or specialty programs Obtaining data on physical, psychological, social and other factors that may influence patient/family health status and share with clinical team Assessing eligibility, and providing updates and recommendations for care planning for new patients to the appropriate clinical team Acting as liaison with clinical supervisors, physicians, and team members on behalf of patients and families Serving as a liaison and educator to community and medical professionals regarding services Required Qualifications Associate degree Licensed or qualified by education, training, or has experience in the performance of selected acts, tasks, or functions 2 years experience in home health, hospice or critical care navigation Knowledge of clinical manifestations of diseases and ability to develop specific clinical knowledge Understanding of specialty programs, and available services for eligible patients Excellent communication, negotiation and public relations skills Excellent presentation and public speaking skills Current and valid Driver's License Reliable transportation and valid automobile insurance coverage Desired Qualifications Accredited practical nurse, LPN, vocational nurse or in the social work or psychology field General Nursing, Social Work or Care Management knowledge preferred Hospice and/or Home Health experience Familiarity with Aprima Electronic Medical Record (EMR) system Location Office Location:

The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
