You will receive referrals, convert them for delivery of services and disseminate information throughout the agency.
In this role you will be responsible for:
Establishing and maintaining effective and efficient systems for patient referrals Answering all inquiries regarding agency services and disseminates appropriate information including referrals to community resources Accurately documenting or tracking all referrals received by the agency for trending reports Being responsive to internal and external customer needs and satisfying their requirements and expectations Greeting all individuals arriving at the office courteously and ensure their needs are met Maintaining organized, effective and efficient systems and communication to ensure the continuity of quality patient care delivered Assisting in evaluating agency operations and systems and identifying ways to enhance workflow and productivity Efficiently and accurately transferring referrals to supervisor or scheduler for patient evaluation Accurately data processing all necessary information into the company database software for immediate use Required Qualifications High School Diploma or GED Complete required work daily Cope with work involving ill, disabled, and terminal patients Problem solving skills Excellent observation, verbal and written communication skills Desired Qualifications Associate degree One (1) year of experience working in a healthcare setting Familiarity with Electronic Medical Record (EMR) systems Location Home Office:
