New patient information is including but not limited to diagnosis history, past family medical social history, pharmacy information, durable power of attorney information, durable power of attorney paperwork, medication list and allergy lists.
Patient On-Boarding is also responsible for ensuring all new patients or authorized representatives are offered Patient Portal credentials.
Patient On-Boarding will manage obtaining any missing documents including outstanding consents or new patient paperwork.
Patient On-Boarding will work to ensure all new patients and families are properly on-boarded into Careline Physician Services.
Responsibilities are completed with limited supervision and may involve medical and social service activities and correspondence.
This position requires initiative, creative problem resolution, accurate data entry skills, excellent phone and electronic communication skills, service excellence and a desire to work within a practice that centers around and with a geriatric population In this role you will be responsible for:
Accurately enters all clinical patient information into the electronic medical records including but not limited to past medical family social history, medication list, allergies, diagnosis history and pharmacy information.
Uses resources available to the practice for appropriate patient sharing platforms to obtain or search for any missing patient diagnosis, surgical, past medical family social history and or allergy information.
Contacts patients, families or facilities to obtain any missing patient history information to ensure chart is appropriately populated for a new patient visit with Careline Physician Services.
Obtains all durable power of attorney, guardianship, do not resuscitate, all required consent forms and other pertinent forms related to legal decisions or decision makers.
Measurement Criteria:
Standard is met when there is no incidence indicating deviance On-boards all new patients or families to ensure the patient and or family understands any additional requirements or needed forms regarding the participation in any advanced payment model program.
Required Qualifications High School Diploma or GED Minimum one (1) year of experience in medical field, medical office experience or related position Ability to operate computer equipment, Fax and photocopy machines necessary Proficient in Microsoft Office Suite, including Outlook, Word, Excel, and PowerPoint Knowledge of medical terminology Excellent verbal and written communication skills Desired Qualifications C.

The check will bounce and the victim is left responsible.
